POST HOSPITALIZATION
Patient care does not end when the patient gets discharged from the hospital. In fact, in our experience, the most crucial part starts there.
The need for skilled home health care is paramount for those patients who have undergone cardiac surgeries, hip and knee replacements, spine surgeries, transplants, tumour removals, and many other kinds of surgeries. Specialized after-discharge care is required for such patients and it is important to follow an evidence-based regime to avoid unnecessary complications and to ensure speedy recovery.
Post Hospitalization Care @ Home
Our post hospitalisation services are designed to help the patient recover completely physically and mentally, and resume his or her normal routine, all the while offering mental support and strength to the patient and the family.
Our Post Hospitalization Home Health Care normally involves a combination of the following:
- Full time / part time nursing care (checking wounds, wound management, suture removal, tracheotomy, oxygen administration, injections, reviewing medications and others)
- Physiotherapy / rehabilitation
- Speech therapy, including help with swallowing
- Patient education and confidence building
- Integral to the post hospitalization care is custodial care that includes assisting the patient with self-grooming and personal tasks such as bathing, dressing and so on.
